We participate with most of the major insurance plans. We have contracts with many PPO (preferred provider organization), POS (point-of-service), and some HMO (health maintenance organization) health plans.
If we don’t have an arrangement with your particular insurance carrier, there is usually only a small additional expense to you. Our staff will help you complete the necessary forms to send to your insurance company.
If your visit is the result of some work-related injury, we’ll accept Workers’ Compensation payments. Naturally we will ask you to provide a claim number and billing information, which you can obtain from your employer.
If your injury or illness has resulted in legal action and you are represented by an attorney, we’ll be happy to work with your attorney. We will provide you with a properly-executed authorization and assignment statement.
Please be assured that your privacy will be completely protected. We will not discuss any information about you, your health, or your financial circumstances with any third party unless you have signed a recent HIPAA-compliant authorization form specifically permitting us to do so. We adhere very strictly to this federal law, and our staff members are carefully trained to guard your privacy. |
